Guidelines for Child and Adolescent Mental health Clinicians who want to refer to Atrom Mindcare for Adult mental health care

If your patient is 17 years old and you are considering transitioning them onto Adult Psychiatric Care- please follow the following pathway.

• Discuss the plan to transition with them and you can provide them with the attached information leaflet.

• When they turn 18 years old, you can refer them to Atrom Mindare Holistic Psychiatric Clinic by emailing us at contact@atrommindcare.com or ringing us on: 02037403978.​

• In order to refer, please provide your patient’s name, parents/guardian’s name and contact details and a recent update on their mental state, their diagnoses and the medications they are on and any other information you think we may find helpful. An initial assessment letter is helpful, particularly if your patient has been diagnosed with ADHD or Autism Spectrum Disorder, so we do not have to repeat this assessment within the Adult service.

• When an appointment with us is confirmed, our secretaries will inform you of the same.
